Writes rows to cohort_table (created if missing) and returns them. replace = TRUE first deletes any
existing rows with the same cohort_definition_id. Vocabulary tables are looked up on the connection
(main schema, then the attached central vocabulary) unless concept_ancestor = / concept = are passed.
